Woodland Clinic’s diabetes program is a multidisciplinary education, assessment and management program, consisting of two 90-minute sessions in a group setting. The goal of the program is to help each patient learn how to effectively manage his or her disease. Throughout the program, patients are given the opportunity to speak with the following professionals:
- Dietitian/nutritionist about diet, meal planning, vitamins, minerals and herbal supplements.
- Diabetic nurse educator about any problems/concerns you may have about diabetes, blood sugar testing, medications, lab work and the latest information on diabetes.
- Medical assistant will assist with any necessary follow-up appointments and lab work.
- Physician about medication concerns, medication response, blood sugar values and your progress.
Sessions are held on Wednesday, 3 p.m. to 4:30 p.m. at the Woodland Clinic, across from Woodland Memorial Hospital. For more information or to register, call 530.668.2605, ext. 6202.
